Release Form

	Texas Salsa Championship Release forms are a requirement for participation. A Release Form must be completed by each participant in order to take part in the championship events including the Master Classes, Beginners Workshops, Exhibition and Competition. Release forms are to be submitted along with full payment by February 25th or with late entries March 4th.
	Texas Salsa Championship Release Form
	I hereby release the teachers, staff, volunteers, promoters, and judges of the Texas Salsa Championship, the Texas Association for Hispanic Dance and Culture, and the all Dallas Independent School District from any and all claims for damages, injuries, or loss to my person or property during my participation in the Texas Salsa Championship. I further release the teachers, staff, promoters, volunteers, and judges from any liability for injury, damage, or loss which may have been caused by any act or omission.
	Permission to Use Photo or Video Image By signing the release form the participant named below gives permission to use his or her photo image or or video recording for promotion of the Texas Salsa Championship, and for promotion and advancement of Hispanic Dance by the Texas Association for Hispanic Dance and Culture.
	By signing, each of the participants agrees to be bound by the waiver and release agreement. Participants have read the rules and regulations of the competition and agree to each. The signature of the parent or guardian is required for all contestants under eighteen years of age. The signing of the agreement is a condition of participation in the Texas Salsa Championship.
