A Release Form must be completed by each participant to take part in festival events including the Free Showcase, Free Workshops, Master Classes, and Competition. |

| I hereby release the teachers, staff, volunteers, promoters, and judges of the Folklórico Festival of Texas, the Texas Association for Hispanic Dance and Culture, and the all Dallas Independent School District from any and all claims for damages, injuries, or loss to my person or property during my participation in the Folklórico Festival of Texas. I further release the teachers, staff, promoters, volunteers, and judges from any liability for injury, damage, or loss which may have been caused by any act or omission. | |
| By signing the release form the participant named below gives permission to use his or her photo image or or video recording for promotion of the Folklórico Festival of North Texas and for promotion and advancement of Hispanic Dance by the Texas Association for Hispanic Dance and Culture. | |
| By signing, each of the participants agrees to be bound by the waiver and release agreement. Participants have read the rules and regulations of the competition and agree to each. The signature of the parent or guardian is required for all contestants under eighteen years of age. The signing of the agreement is a condition of participation in the Folklórico Festival of Texas. | |
